BASIC EDUCATION ADMISSION REQUIREMENTS

  1. Completely filled out Basic Education Admission Application Form.
  2. Original High School Report Card/Form 137 or Form 138
  3. Certificate of Good Moral Character
  4. Pictures: 1x1 and 2x2 colored with white background (3pcs each)
  5. Photocopy of Birth Certificate or legal adoption papers (original copy will be presented for verification)
  6. Plastic and long brown envelopes (1 pc of each)

COLLEGE ADMISSION REQUIREMENTS

  3.  Transcript of record (Form-137)
  4. Certificate of Good Moral Character
  5. NSO (authenticated copy of Birth Certificate

Senior High School ADMISSION REQUIREMENTS

Senior high school or SHS refers to the last two years of the K to 12 program that has been implemented by the Department of Education here in the Philippines .

  5. Moving Up Certificate
  6. QVR Certificate (From private school)

  9. Long brown Envelope and Plastic Enveloped
